1. A parent or legal guardian must accurately complete all information on this application form. Attach additional pages if necessary.
2. Attach the following required documents:

• The most recent year’s tax form.
• a personal reference form completed by an adult who is not a member of the family attesting to the applicant’s eligibility for the financial aid (attached) 

3. Failure to provide all of the necessary documentation requires the Financial Aid Committee to deny the request for aid.
4. Include all signatures and dates on the application and personal reference forms.

SCSA Financial Assistance

1. Financial aid will be in the amounts of $100, $250 or $500, depending on need. Aid is granted per season (spring and fall for U14 and younger is considered two seasons). A new application must be submitted for the second season if aid is still being requested. The application for the second season will be an abbreviated version of the first. All other miscellaneous team fees are the responsibility of the applicant or team.
2. Family members will be asked to assist SCSA in various activities serving as volunteers.
3. There must be a true, verifiable financial need.
4. There must be evidence of good character.
5. A personal interview may be required of the applicant or a family member.
6. The committee will render a decision as soon as possible upon receipt of all necessary information. The decision of the Financial Aid Committee is final. 
7. Information contained in this application is considered confidential by SCSA board, committee members, team coaches or manager and the parties making application for financial aid.
8. All financial assistance is credited to the team for which the applicant intends to play and all balances remaining must be paid by the posted dates.
